Wholesale Order Form

A wholesale order form is a trade document. It needs the account reference, a purchase order number, quantities per line, and a delivery window - because a shop buyer's warehouse has opening hours.

For online shops, retailers and independent brands.

PO number and delivery window, or the order sits in a bay

Retail buyers cannot pay an invoice without a purchase order number, and their goods-in team cannot accept a pallet outside a booked window. Both are trivial to ask for and catastrophic to omit.

So this template makes the PO number and the delivery window part of the order rather than an afterthought. It also asks for the account reference, which lets you apply the right price list without a conversation about terms.

"My order is wrong" with no order number is a support conversation that takes four emails. Asking for the number, the item and what happened turns the same message into something one person can resolve on the first reply.

Can this handle a full line sheet?

For a few dozen lines, yes - use quantity fields per line or one structured text box. Beyond that most brands send a spreadsheet and use the form for the account, PO and delivery details.

Can I show trade prices?

Put them in the option labels, or link to a price list from the introduction. Because the form is a normal page, you can also keep the link unlisted and share it only with approved accounts.

Can I take payment on the form?

Not yet. Most people use this to capture the order and then invoice or take payment on collection - which also means you can confirm availability before charging anyone.

Can I limit what people can order?

Yes. Options are lists you control, so only what you actually sell appears - and you can close the form when you're at capacity.

Will I be told about new orders straight away?

Yes, if you turn on live notifications: each order is emailed to you as it arrives. Otherwise they collect in your dashboard.

Can this work alongside my store platform?

Yes - it's a normal link, so it goes on a product page, in a packing slip, in an order confirmation email or in your help centre. Responses stay in ava42, where you can export them.
