Return Request Form
A return request form turns your most annoying support thread into a single submission. Order number, which items, why, and what they want back - collected once, in a list you can work through.
For online shops, retailers and independent brands.
Reason codes tell you what to fix
Individually, a return is a cost. Collected as a list with structured reasons, returns are the best product feedback you will ever get for free - and "too small" appearing forty times against one item is a sizing guide problem you can fix.
That only works if the reason is a dropdown rather than a paragraph. This template uses fixed reasons with a free-text field alongside, so you get both the data and the detail.
The same facts, every time
When each report is written differently, patterns are invisible. Consistent fields mean you can look across a month and see the thing that keeps happening, rather than a folder of prose.
Order numbers turn a complaint into a resolution
"My order is wrong" with no order number is a support conversation that takes four emails. Asking for the number, the item and what happened turns the same message into something one person can resolve on the first reply.
It's the same trick for wholesale enquiries and returns: ask the two facts that decide the answer, and stop the thread before it starts.
Easy enough to actually be used
A reporting process nobody follows produces no data. This one is a link that opens instantly on a phone and takes a couple of minutes, which is the only version that gets used on a busy day.
Edit it like a document, publish like a link
Add questions, remove questions, change the order and the wording, set your accent colour. When you save, the form is live at its own address - share it by link, QR code or a button on your site, and read the answers in your dashboard.

Questions
Can I generate a returns label from this?
No - this collects the request. Most shops reply with a label or an address, and the structured request means that reply can be sent without asking anything else.
Should I ask for a photo of a damaged item?
Ask for a link, and consider making damage claims a separate reason so they route to someone who can approve a replacement immediately.
Can reports be submitted anonymously?
Yes, if you make the name optional. Nothing about the submitter is recorded otherwise - no IP address, no account, no tracking.
Can this go to more than one person?
Yes. Notification recipients are a list, so a report can reach a manager and a safety lead at the same time.
Can I export everything for an audit?
Yes - one click gives you a CSV of every response with its timestamp, which is usually exactly what an auditor or insurer asks for.
Can this work alongside my store platform?
Yes - it's a normal link, so it goes on a product page, in a packing slip, in an order confirmation email or in your help centre. Responses stay in ava42, where you can export them.
