Gift Card Order Form
A gift card order form has two people in it: the buyer and the recipient. Getting both sets of details right, plus the date it should arrive, is the whole job.
For online shops, retailers and independent brands.
The delivery date is the feature
Gift cards are bought early and needed on a specific day. A card that arrives three weeks before a birthday spoils the surprise; one that arrives the day after is worse.
So this template asks for the send date explicitly, and asks whether it should go to the recipient or to the buyer to hand over. Those two answers cover almost every gift-card support question you'd otherwise get, and they cost the buyer ten seconds.
Orders you don't have to decode
Orders taken by phone, text and social message all arrive in different shapes, and the missing detail is always the one you need to start. Then you're calling back, and they're not answering.
One form means every order arrives complete, in the same shape, with a written record of exactly what was asked for.
Order numbers turn a complaint into a resolution
"My order is wrong" with no order number is a support conversation that takes four emails. Asking for the number, the item and what happened turns the same message into something one person can resolve on the first reply.
It's the same trick for wholesale enquiries and returns: ask the two facts that decide the answer, and stop the thread before it starts.
A link you can put anywhere
Website, bio, QR code on the counter, reply to a message. Same form, same list, whichever way the order arrives.

Questions
Can I take payment on the form?
Not yet. Most shops take the order here and send a payment link or invoice, which also lets you confirm the amount and delivery date before charging.
Can the message be printed?
Ask for it as a text question, as this template does, and it's stored with the order exactly as typed - which is much safer than transcribing it from an email.

Yes, if you turn on live notifications: each order is emailed to you as it arrives. Otherwise they collect in your dashboard.
Can this work alongside my store platform?
