Product Warranty Claim Form

A product warranty claim form needs three facts before anything else: proof of when it was bought, which unit it is, and what it's doing. Without those a claim can't be assessed, only discussed.

Serial number and purchase date, or it isn't a claim

Warranty decisions rest on two data points. Everything else in a claim email is narrative, and the narrative arrives without either of them.

Making them required changes the whole process: claims arrive complete, they can be assessed in one pass, and the customer gets a decision on the first reply rather than a request for more information. Asking what resolution they want - repair, replacement or refund - saves the second round.

What if the customer has lost their receipt?

Ask for the order number or the retailer and approximate date, as this template does. Most claims can be verified from your own records if you have either.

Can customers send photos or video of the fault?

Yes - a file question accepts up to three images at 20 MB each. In practice a good written description plus the serial number resolves most claims without images.

Can reports be submitted anonymously?

Yes, if you make the name optional. Nothing about the submitter is recorded otherwise - no IP address, no account, no tracking.

Can this go to more than one person?

Yes. Notification recipients are a list, so a report can reach a manager and a safety lead at the same time.

Can I export everything for an audit?
