Capacity is what separates a supplier from a maker
Plenty of small brands can produce fifty units. Far fewer can produce fifty units a month, every month, with a lead time you can plan around - and the difference only shows up after you've listed them.
So this template asks about monthly capacity and lead time alongside wholesale price and minimum order. Those four answers tell you whether a range is stockable at all, before you spend a meeting falling in love with the product.
Screen before you spend time
Most of the work in an application process is on the ones that were never eligible. Asking the disqualifying questions first - dates, requirements, basic facts - means you spend your reading time on the real candidates.
Order numbers turn a complaint into a resolution
"My order is wrong" with no order number is a support conversation that takes four emails. Asking for the number, the item and what happened turns the same message into something one person can resolve on the first reply.
It's the same trick for wholesale enquiries and returns: ask the two facts that decide the answer, and stop the thread before it starts.
Long enough to decide, short enough to finish

Questions
What certifications should I ask about?
Whatever your category requires - food safety, testing, allergen declarations, sustainability claims. Ask, and ask for expiry dates, because that's the field that goes stale.
Can vendors send a line sheet?
Ask for a link to it. Most brands have one as a PDF in cloud storage, which is easier for both sides than an attachment.
Can applicants attach a CV or document?
Not yet - file uploads are on the roadmap. Most people ask for a link (to a portfolio, a CV in cloud storage, or a profile) which works today and keeps the form fast.
Can I close applications on a deadline?
Yes. Close the form and anyone with the link sees a short closed page instead of the questions, so nobody fills in an application that can't be considered.
Can more than one person review them?
Yes - invite your team into the same account and they'll see the same responses. You can also export to a spreadsheet and score them wherever you like.
Can this work alongside my store platform?
