Wording that tries to sign away all liability is often the least enforceable part of a waiver, and it's never the part that helps on the day something goes wrong. What helps is knowing about the heart condition and having a number to call.

Will this hold up legally?
That depends on your jurisdiction, your activity and your wording - so have a lawyer or your insurer read it. What we can say is that you'll have a dated record of exactly what each person agreed to.
Can I use one waiver for different activities?
Better not to. Generic risk wording is the weakest kind; duplicate the form per activity and name the risks that actually apply.
Is this legal advice?
No. The wording is a practical starting point, not a legal document - have your own adviser check anything you rely on, especially where health, minors or money are involved.
Is a tick box a valid signature?
In many places a clear, dated, affirmative action is accepted, but it depends on your jurisdiction and what you're doing. If you need a drawn signature, print the response and sign it, or take the form as the pre-appointment record.
Can I keep a copy for the person who filled it in?
