A file upload form collects documents and photos alongside the answers that explain them. What matters is that it works everywhere: it is a native file input in a native form post, so it uploads on an old phone with scripting off.

What the file is *for* matters as much as the file
An inbox full of attachments named IMG_4821.jpg is not a process. Without a name against it, a reference, and one line about what you're looking at, every upload starts a conversation to work out whose it is and why it arrived.
So this template asks the identifying questions first and the file last, which is also the order that gets forms finished - people abandon at the upload step more than anywhere else, and by then you already have their details.
Orders you don't have to decode
Orders taken by phone, text and social message all arrive in different shapes, and the missing detail is always the one you need to start. Then you're calling back, and they're not answering.

Questions
How big can a file be, and how many?
20 MB per file on every plan including Pro, up to three files and 30 MB per response. The limit is the same for paying customers on purpose: an upload sits in memory while it's checked, so a limit that protects the service has to apply to everyone.
What file types are accepted?
Images (jpg, png, gif, webp, heic), PDF, Word, Excel, PowerPoint, CSV and plain text. Every upload is checked twice - the extension must be allowed and the file's first bytes must match what it claims to be - so a .png that is really an executable is refused before it reaches you.
Can I take payment on the form?
Not yet. Most people use this to capture the order and then invoice or take payment on collection - which also means you can confirm availability before charging anyone.
Can I limit what people can order?
Yes. Options are lists you control, so only what you actually sell appears - and you can close the form when you're at capacity.
Will I be told about new orders straight away?
Yes, if you turn on live notifications: each order is emailed to you as it arrives. Otherwise they collect in your dashboard.
How many forms can I have?
One on the free plan, with ten responses. Pro is $10 a month for unlimited forms and unlimited responses - one plan, no tiers, no per-response pricing.
