Lead Capture Form
A lead capture form has one number to optimise: completions. Every field you add costs you leads, so each one has to earn its place by changing what happens next.
For small businesses and independent professionals.
Add a field only if it changes your next action
The test for any field on a lead form is simple: if the answer wouldn't change who picks it up or what they say, delete it. Company size, job title, industry - useful to a CRM, invisible to your actual response.
This template keeps three required fields and one qualifying question, and puts everything else as optional. It also forwards the page's query string with the submission, so a campaign parameter on the link is stored with the lead - tracking without a tracking script.
Route it once, correctly
Asking what the message is about is the difference between an inbox and a queue. One dropdown means the right person can pick it up without three forwards.
The forms every business needs, without the software
Most businesses need four or five forms and get sold a platform with forty features. The result is a monthly bill, a builder to learn and a page that takes three seconds to appear.
This is the opposite: one template, edited in a minute, published as a page that loads in one request. Free for your first form, and $10 a month if you need more than one.
Answers in one place
Every message lands in the same list, timestamped and searchable, and can be emailed to you as it arrives. Nothing lost in a shared inbox.
One minute to make it your own
Every label, option and placeholder on this template is editable, and you can add whatever this template doesn't ask. Save to publish, share the link, and export the responses whenever you need them in a spreadsheet.

Questions
How do I know which campaign a lead came from?
Add a parameter to the link - one per ad, page or email - and it's submitted with the response. No cookies, no third-party pixel, and it works with JavaScript off.
Should I gate content behind this?
If you do, keep it to name and email. Every extra field on a gate costs you more leads than the data is worth, and people type nonsense into the ones they resent.
Will I get spam?
Far less than a published email address. Every form carries a hidden trap field that automated submitters fill in and humans never see, and those submissions are silently discarded.
Can I send messages to a shared inbox?
Yes. Notifications go to a list of addresses you choose, so a team alias works as well as one person.
Can I add this to my website?
Link to it from anywhere - a button, a nav item, a footer. It's a normal page with its own URL, so no embedding or code is required.
How many forms can I have?
One on the free plan, with ten responses. Pro is $10 a month for unlimited forms and unlimited responses - one plan, no tiers, no per-response pricing.
