Consent Form
A consent form is only worth having if it records a real decision. That means separate permissions, plain wording, and a date you didn't have to write down yourself.
For small businesses and independent professionals.
One tick per decision
The single "I agree to all of the above" box is the most common mistake in consent, and the least defensible. Someone who was happy with two of five things has no way to say so, so they tick everything or nothing - and you have a record of neither.
Every permission here is its own box, worded as one specific thing. Each submission is stored with the exact time it arrived, so the record is the submission itself rather than a note somewhere saying consent was given.
Say what they're agreeing to, in plain words
A consent form that nobody reads protects nobody. The statements in this template are short, specific and separate - so ticking one is a real decision rather than a formality.
Edit the wording to match what you actually do. It should sound like your practice, not like a legal template.
The forms every business needs, without the software
Most businesses need four or five forms and get sold a platform with forty features. The result is a monthly bill, a builder to learn and a page that takes three seconds to appear.
This is the opposite: one template, edited in a minute, published as a page that loads in one request. Free for your first form, and $10 a month if you need more than one.
Digital, dated, and searchable
Every response is stored with the date and time it was submitted, and the whole set exports as a spreadsheet. Finding one person's answers takes seconds rather than a filing cabinet.
Edit it like a document, publish like a link
Add questions, remove questions, change the order and the wording, set your accent colour. When you save, the form is live at its own address - share it by link, QR code or a button on your site, and read the answers in your dashboard.

Questions
Does a tick box count as a signature?
In many jurisdictions a clear, dated, affirmative action is accepted - but it depends where you are and what you're doing. For anything that matters, have a lawyer read your wording.
Can I prove when consent was given?
Every response carries the timestamp it was submitted, visible in your dashboard and included in exports. That's usually what's asked for.
Is this legal advice?
No. The wording is a practical starting point, not a legal document - have your own adviser check anything you rely on, especially where health, minors or money are involved.
Is a tick box a valid signature?
In many places a clear, dated, affirmative action is accepted, but it depends on your jurisdiction and what you're doing. If you need a drawn signature, print the response and sign it, or take the form as the pre-appointment record.
Can I keep a copy for the person who filled it in?
Yes - they see a confirmation on screen, and you can turn on an email so a copy of each submission lands in your inbox for your records.
How many forms can I have?
One on the free plan, with ten responses. Pro is $10 a month for unlimited forms and unlimited responses - one plan, no tiers, no per-response pricing.
