General Business

Survey Form

A survey form is a research instrument, and the main thing that ruins one is length. Ten good questions answered by three hundred people beats forty answered by twenty.

For small businesses and independent professionals.

Every extra question costs you responses

Drop-off in surveys is brutal and non-random: the people who abandon are the busy ones, which skews your results towards whoever had a slow afternoon. Length is the single biggest cause.

So this template uses fast question types - scales you tap once, a ranking you drag, fixed options - and reserves open text for two questions. It also uses conditional logic so follow-ups appear only for the people they apply to, which keeps the form short for everyone else.

Ask while they still remember

Feedback collected a week later is a memory of a feeling. Asked the same day, it's specific - which is the only kind you can act on.

Short, fast and answerable from a phone is what gets a reply. This template is built for a two-minute answer, not a survey.

The forms every business needs, without the software

Most businesses need four or five forms and get sold a platform with forty features. The result is a monthly bill, a builder to learn and a page that takes three seconds to appear.

This is the opposite: one template, edited in a minute, published as a page that loads in one request. Free for your first form, and $10 a month if you need more than one.

Two minutes, on a phone, after the fact

Send the link by text or print a QR code on the receipt. The page opens instantly, works without an app and submits in one request - the difference between a response rate and a rounding error.

Edit it like a document, publish like a link

Add questions, remove questions, change the order and the wording, set your accent colour. When you save, the form is live at its own address - share it by link, QR code or a button on your site, and read the answers in your dashboard.

Questions

Does the ranking question work on a phone?

Yes - drag to order with a finger. Without JavaScript it degrades to a number dropdown per option, so it still works and still records a full ranking.

How do I analyse the results?

Export to CSV and use whatever you already have. Scales and rankings come out as numbers, so a pivot table or a chart is a couple of clicks.

Can responses be anonymous?

Yes. Make the name and email optional (or delete them) and people can answer without identifying themselves. Nothing else about the visitor is recorded - no IP address, no tracking.

Can I see averages over time?

Export the responses and you have every rating with its date, ready to chart. Each response is also readable on its own in your dashboard.

How do I get people to fill it in?

Short beats clever: ask one rating and one open question, send the link the same day, and don't make anyone log in. Most of the drop-off is length, not interest.

How many forms can I have?

One on the free plan, with ten responses. Pro is $10 a month for unlimited forms and unlimited responses - one plan, no tiers, no per-response pricing.
