Pool Service Request Form
A pool service request form needs the volume and the plant. Size sets the chemical dosing; the pump, filter and heater tell you what can actually be serviced or fixed.

A pool that's clear but under-chlorinated needs a routine service. A green pool needs a recovery programme, several visits and a conversation about cost - and the two look identical in a booking that just says "pool service".
So this template asks about the water's current state, when it was last serviced, and whether it's been left covered over winter. Those answers separate a maintenance round from a rescue job before you commit a slot to it.
Fewer no-shows, fewer surprises
No-shows and "I thought I booked the other thing" both come from the same place: a booking nobody wrote down properly. When the customer types it themselves, there's a record, in their words, of exactly what was asked for.
Address, access and "is anyone home?"
The details that decide whether a visit is 40 minutes or two hours are all logistical: the postcode, the parking, how many rooms, whether there's a dog, who's letting you in.

Questions
Should I ask for equipment makes and models?
Ask for pump and filter make at least - parts are model-specific and a return visit for a seal costs more than the seal. A photo of the plant room is even better.
Can this handle both domestic and commercial pools?
Ask which, as the first question. Commercial pools carry testing and record-keeping obligations that change the service entirely, so many companies run two forms.
Does this book the slot automatically?
No - it collects a request, and you confirm it. That's deliberate: real diaries have exceptions, and a form that promises a slot you can't honour costs more than one that asks.
Can I limit the options people can pick?
Yes. The service and time-of-day lists are yours to edit, so people can only choose from what you actually offer. You can also close the form when you're full.
Will I know straight away?
Yes, if you want to be. Turn on live notifications and every request is emailed to you the moment it arrives; otherwise collect them and read them in your dashboard.
Can I ask for a postcode and check it's in my area?
You can ask for it, and it's stored with the response - so you'll see immediately whether a job is in range. Automatic area checking isn't built in; a line in the form saying which areas you cover does the job.
