Handyman Service Request Form
A handyman service request form has to handle a list, not a job. Most requests are five small things, and what you need to know is how long the whole list takes and who's buying the materials.

Ask for the whole list, then who buys the parts
Handyman work is quoted in half-days, so the useful question isn't "what's the job?" - it's "what's everything on the list?". A customer who mentions two tasks and produces four more on the day has broken your schedule, not just your quote.
The second question is materials. "I assumed you'd bring it" is the most common friction in this trade, and it's solved by one dropdown asked before anyone turns up with an empty van and a full list.

Questions
How do I stop the list growing on the day?
Quote against the written list and say so - the form is your record of what was agreed. Extra items become a second visit or an extra hour, priced there and then.
Should I offer a minimum charge?
Most do - a minimum call-out or a half-day rate. Say it on the form and ask for an acknowledgement; nobody enjoys hearing it on the doorstep.
Can I ask for photos or files?
Not yet - uploads are on the roadmap. In the meantime, most people ask for a link to photos, or reply to the response with an email asking for them. Everything else about the job is captured here.
Can I see where each quote request came from?
Yes. Add a hidden tracking parameter to the link (for example one link on your site and another on your Google profile) and it's stored with the response.
