Massage involves undressing and touch, which makes clear boundaries a professional necessity rather than an awkwardness. Most clients have no idea what to expect: what to remove, how draping works, whether they can ask for something to stop.

Is it necessary to spell out draping?
It genuinely helps. First-time clients worry about it and won't ask, and a therapist who has written it down is a therapist who has thought about it.
Should this be re-signed?
Once, then a shorter health update at each visit. What changes between appointments is health, not consent to the treatment itself.
Is this legal advice?
No. The wording is a practical starting point, not a legal document - have your own adviser check anything you rely on, especially where health, minors or money are involved.
Is a tick box a valid signature?
In many places a clear, dated, affirmative action is accepted, but it depends on your jurisdiction and what you're doing. If you need a drawn signature, print the response and sign it, or take the form as the pre-appointment record.
Can I keep a copy for the person who filled it in?
