Massage Intake Form

A massage intake form should tell a therapist what to work on and what to avoid, before the client is face down and unable to have a detailed conversation.

For spas, massage therapists and wellness clinics.

Pressure preference, asked properly

"Is the pressure okay?" asked mid-massage gets a yes from almost everyone, because the client is relaxed, face down, and doesn't want to be difficult. Then they're sore for two days and don't rebook.

Asked on a form beforehand, people are much more honest - and offering a scale rather than "light, medium or firm" gets a more useful answer still. This template also asks which areas to avoid entirely, which is the question that prevents the most complaints.

What a good intake form saves you

The first message from a new client almost never contains what you need. You reply asking one question; they answer a different one; a week later you still can't start. Every round trip is a day, and the clients who go quiet are usually the ones who got tired of answering.

One form ends that. Ask everything once, get a complete picture, and start the same day. The questions in this template are the ones you end up asking anyway - the form just gets them asked up front.

Health screening that protects the treatment

Pregnancy, recent surgery, medication, injuries, pressure preference - the questions that decide whether a treatment is safe and whether it will be enjoyed. Asked at the door they get a shrug; asked in advance they get an answer.

The form is also where you set expectations: arrival time, cancellation terms, what to expect. Written down once, it stops most of the awkward conversations at the desk.

Fast enough that people finish it

Abandonment is mostly a speed problem. A form that takes a second to appear and submits in one request gets finished; one that loads a framework first does not. This is a single self-contained page - no scripts to download, nothing to wait for.

Edit it like a document, publish like a link

Add questions, remove questions, change the order and the wording, set your accent colour. When you save, the form is live at its own address - share it by link, QR code or a button on your site, and read the answers in your dashboard.

Questions

How much medical history do we need?

Enough to know what to avoid: recent injuries, surgery, blood clots, pregnancy, medication that affects bruising or sensation. Not a full history.

Should we ask about pregnancy?

Yes, and the trimester - it changes positioning entirely and rules out some techniques. Ask it separately from other conditions.

Can I add or remove questions?

Yes - every question here is a starting point. Add your own, delete what doesn't apply, reorder by dragging, and make anything required or optional. Saving publishes the change immediately.

Where do the answers go?

Into your dashboard, where you can read, filter and export them as a spreadsheet. You can also have every response emailed to you the moment it arrives, or as a daily or weekly digest.

Do I need a website?

No. Every form gets its own link you can paste into an email signature, a text message, a QR code or a social profile. If you do have a site, you can link to it from anywhere on it.

Can I keep these as treatment records?

Yes - each response is dated and exportable, which is what most insurers want to see. Ask only what's clinically relevant, and check your professional body's guidance on how long to keep it.
