Home actives are the contraindication nobody declares
A client using a prescription retinoid, or a strong acid three nights a week, has a skin barrier that behaves nothing like it looks. A standard exfoliating facial on that skin can strip it, and the reaction appears the next day.
Clients don't withhold this - they genuinely don't know it counts, because it's a cream from a shop or a prescription for something unrelated. So this template asks specifically, by name, with a short explanation of why.
Before the appointment, not in the waiting room
Paperwork at the door costs you the first ten minutes and gets rushed. Send the link when you confirm the booking and it arrives filled in, read properly, with time to flag anything that needs a conversation.
Health screening that protects the treatment
Pregnancy, recent surgery, medication, injuries, pressure preference - the questions that decide whether a treatment is safe and whether it will be enjoyed. Asked at the door they get a shrug; asked in advance they get an answer.
The form is also where you set expectations: arrival time, cancellation terms, what to expect. Written down once, it stops most of the awkward conversations at the desk.

Questions
How long should clients pause actives before a facial?
Follow your product guidance - commonly 3 to 7 days for acids and retinoids. The point of the question is finding out in time to advise them.
What about isotretinoin?
Most protocols require a six to twelve month gap after finishing a course before resurfacing treatments. Ask about it explicitly - it's the single most important question on a facial consent form.
Is this legal advice?
No. The wording is a practical starting point, not a legal document - have your own adviser check anything you rely on, especially where health, minors or money are involved.
Is a tick box a valid signature?
In many places a clear, dated, affirmative action is accepted, but it depends on your jurisdiction and what you're doing. If you need a drawn signature, print the response and sign it, or take the form as the pre-appointment record.
Can I keep a copy for the person who filled it in?
Yes - they see a confirmation on screen, and you can turn on an email so a copy of each submission lands in your inbox for your records.
Can I keep these as treatment records?
Yes - each response is dated and exportable, which is what most insurers want to see. Ask only what's clinically relevant, and check your professional body's guidance on how long to keep it.
