The health section keeps a client safe. The preferences section is what makes them come back: the client who hates having their scalp touched, the one who wants silence, the one who is always cold, the one whose feet are ticklish.

How often should the intake be refreshed?

Annually, and a short health check at each visit. Re-sending the same link builds a dated history rather than overwriting what you knew.

Can we store this long-term?

Yes - responses stay in your account with their date, and export as a spreadsheet. Tell clients what you keep and for how long, as your local rules require.

Can I add or remove questions?
