Spa Gift Card Order Form

A spa gift card order form has two people in it and one date that matters. The buyer, the recipient, and the day it needs to arrive.

For spas, massage therapists and wellness clinics.

A gift card for a treatment, not just an amount

Spa gift cards sell better as an experience than a number. "A 60-minute massage and access to the spa" is a present; "£75" is a voucher. The buyer usually wants the first and is offered the second.

So this template offers both, and asks which treatment if they choose an experience. It also asks about the delivery date and whether it goes to the recipient or the buyer - the two questions that account for almost every gift card support message.

Orders you don't have to decode

Orders taken by phone, text and social message all arrive in different shapes, and the missing detail is always the one you need to start. Then you're calling back, and they're not answering.

One form means every order arrives complete, in the same shape, with a written record of exactly what was asked for.

Health screening that protects the treatment

Pregnancy, recent surgery, medication, injuries, pressure preference - the questions that decide whether a treatment is safe and whether it will be enjoyed. Asked at the door they get a shrug; asked in advance they get an answer.

The form is also where you set expectations: arrival time, cancellation terms, what to expect. Written down once, it stops most of the awkward conversations at the desk.

A link you can put anywhere

Website, bio, QR code on the counter, reply to a message. Same form, same list, whichever way the order arrives.

One minute to make it your own

Questions

Should gift cards be for a value or a treatment?

Offer both. Experience cards convert better and have a clearer perceived value; value cards suit buyers who don't know what the recipient likes.

How long should they be valid?

State it plainly on the form. Long expiry is kinder and better for your reputation, and in some places short expiry periods are restricted - check your local rules.

Can I take payment on the form?

Not yet. Most people use this to capture the order and then invoice or take payment on collection - which also means you can confirm availability before charging anyone.

Can I limit what people can order?

Yes. Options are lists you control, so only what you actually sell appears - and you can close the form when you're at capacity.

Will I be told about new orders straight away?

Yes, if you turn on live notifications: each order is emailed to you as it arrives. Otherwise they collect in your dashboard.

Can I keep these as treatment records?

Yes - each response is dated and exportable, which is what most insurers want to see. Ask only what's clinically relevant, and check your professional body's guidance on how long to keep it.
