Spa Appointment Form
A spa appointment form has to build a sequence, not book a slot. Two treatments back to back need the right therapist, the right rooms and the right order.
For spas, massage therapists and wellness clinics.
Treatment order and room turnaround are the real constraint
A facial then a massage is straightforward. A body wrap then a facial needs a shower in between and a room that's been cleaned. Spas lose time when a booking is taken as "90 minutes" without regard to what has to happen between the two halves.
So this template asks which treatments and whether they'd like them consecutively, plus a therapist preference. It also asks the health flags up front, because a pregnancy or a recent injury changes which treatments can even be offered.
Stop booking by phone tag
Half your clients would rather book at 10pm than call at 10am, and the ones who do call reach a voicemail while you're working. Every missed call is a booking that went somewhere open.
A request form takes bookings while you're busy, asleep or driving - with the details you need written down rather than remembered.
Health screening that protects the treatment
Pregnancy, recent surgery, medication, injuries, pressure preference - the questions that decide whether a treatment is safe and whether it will be enjoyed. Asked at the door they get a shrug; asked in advance they get an answer.
The form is also where you set expectations: arrival time, cancellation terms, what to expect. Written down once, it stops most of the awkward conversations at the desk.
It's a request, not a calendar takeover
You stay in control: requests arrive, you confirm the ones that work. No syncing, no double-booking, nothing to install - just the details you need in the order you need them.
Make it yours in a minute
Change the wording, drop the questions you don't need, add the ones you do, put your own colour on it. Saving publishes it, and you get a link to paste into your website, your email signature or a message. Responses land in your dashboard and download as a spreadsheet whenever you want them.

Questions
Should clients choose their therapist?
Offer it with "first available" first. It fills the diary better and tells you which therapists are in demand - useful information at review time.
Do I need the health questions at booking?
A short version, yes - enough to know whether the treatment is possible. The full intake form can follow once the appointment is confirmed.
Does this book the slot automatically?
No - it collects a request, and you confirm it. That's deliberate: real diaries have exceptions, and a form that promises a slot you can't honour costs more than one that asks.
Can I limit the options people can pick?
Yes. The service and time-of-day lists are yours to edit, so people can only choose from what you actually offer. You can also close the form when you're full.
Will I know straight away?
Yes, if you want to be. Turn on live notifications and every request is emailed to you the moment it arrives; otherwise collect them and read them in your dashboard.
Can I keep these as treatment records?
Yes - each response is dated and exportable, which is what most insurers want to see. Ask only what's clinically relevant, and check your professional body's guidance on how long to keep it.
