Do we need a prescriber to review this?
For anything prescription-only, yes - and the form should say a consultation with a prescriber is required. Requirements differ by country; check yours before offering treatments.
Should we ask about body dysmorphia?
Ask what they want changed and how long they've felt that way, and be prepared to decline. Practitioners who never turn anyone away are the ones who end up in difficulty.
