Tournament Registration Form
A tournament registration form is filled in by a team manager and has to produce a draw. Division, squad size, and whether they can play across the whole day.
For clubs, teams, leagues and community sport organisers.
A draw needs divisions confirmed before it can be made
Tournament organisers can't publish a schedule until they know how many teams are in each division. Registrations that say "we'll bring a team" without confirming the division leave the draw blocked, usually until two days before.
So division and squad size are required here, alongside availability across the day - because a team that has to leave by 2pm cannot be in a final. It also asks for a manager's mobile, which is the only field that matters once the day starts.
One list, not five threads
Registrations that arrive by email, text and word of mouth become a headcount nobody trusts. You find out on the day that two people thought they were signed up and one wasn't.

Questions
Should I collect the full squad list?
For anything with eligibility rules, yes - names and dates of birth. For casual tournaments, a headcount and a manager is enough.
How do I handle late withdrawals?
Ask for a deposit and state the deadline. A team that withdraws the night before leaves a hole in a draw that can't be filled.
Can I stop registrations when we're full?
Yes. Close the form and the link shows a short closed page, so nobody registers for a place that doesn't exist. Reopen it in one click if space frees up.
Can I collect one registration per person or per group?
Either. Many people ask for the group size and the names in one text box; others publish the link and ask everyone to fill it in individually.
Can I get the list as a spreadsheet?
Yes - one click exports every response as CSV, which opens in Excel, Numbers or Google Sheets.
Can parents fill this in for a child?
Yes - that's the normal case. Ask for the player's details and the parent's contact details on the same form, and keep the consent boxes for the adult to tick.
