Martial Arts Registration Form

A martial arts registration form needs a grade and an injury history. Both determine which class a student can safely join and how much contact is appropriate.

For clubs, teams, leagues and community sport organisers.

Consent to contact level, explicitly

Martial arts involve deliberate physical contact, and the expected level varies enormously between styles and within a club. A student expecting light drills who is put into hard sparring gets hurt or never comes back.

So this template asks explicitly what level of contact the student consents to, and asks separately for under-18s where the parent decides. It also asks about grade in the student's own system - because a black belt in one art is a beginner in another and both need saying.

One list, not five threads

Registrations that arrive by email, text and word of mouth become a headcount nobody trusts. You find out on the day that two people thought they were signed up and one wasn't.

Questions

How do we handle grades from other clubs?

Ask for the style, the grade and the awarding body. Most instructors assess in person anyway, but knowing the background avoids an awkward first session.

Should parents decide the contact level for children?

Yes, and record it. It's the single most useful safeguarding field on a martial arts form, and it protects the instructor as much as the child.

Can I stop registrations when we're full?

Yes. Close the form and the link shows a short closed page, so nobody registers for a place that doesn't exist. Reopen it in one click if space frees up.

Can I collect one registration per person or per group?

Either. Many people ask for the group size and the names in one text box; others publish the link and ask everyone to fill it in individually.

Can I get the list as a spreadsheet?

Yes - one click exports every response as CSV, which opens in Excel, Numbers or Google Sheets.

Can parents fill this in for a child?

Yes - that's the normal case. Ask for the player's details and the parent's contact details on the same form, and keep the consent boxes for the adult to tick.
