Baseball Registration Form
A baseball registration form has to place a player in the right division and work out what equipment they own. Age, experience, preferred position and whether they have a glove.
For clubs, teams, leagues and community sport organisers.
Ask what equipment they already have
Baseball needs kit, and the families most likely to have none are the ones a club most wants to reach. A registration that assumes a glove and cleats quietly excludes them, because nobody wants to be the parent who asks.
So this template asks what a player already has as an ordinary logistical question. Clubs that ask consistently find they can run a loan box - and a child who borrowed a glove for a season is usually a player for years.
One list, not five threads
Registrations that arrive by email, text and word of mouth become a headcount nobody trusts. You find out on the day that two people thought they were signed up and one wasn't.
This template makes one list, with the same details for every person, in the order they registered.

Club admin happens in evenings and car parks. Whatever collects registrations, availability and subs has to work in thirty seconds on a phone, for a parent who has never heard of it and will never make an account.
One link does that. Emergency contacts, medical notes and consents come back in writing, and the list exports straight into the spreadsheet the club already runs on.

Questions
How do we handle age divisions?
Ask for a date of birth rather than an age - divisions are usually set by a cut-off date, and parents get the calculation wrong. Let the form collect the fact and do the maths yourself.
Should we ask about position?
Ask for a preference and note that coaches decide. It tells you whether you have four catchers and no pitchers before the first practice.
Can I stop registrations when we're full?
Yes. Close the form and the link shows a short closed page, so nobody registers for a place that doesn't exist. Reopen it in one click if space frees up.
Can I collect one registration per person or per group?
Either. Many people ask for the group size and the names in one text box; others publish the link and ask everyone to fill it in individually.
Can I get the list as a spreadsheet?
Yes - one click exports every response as CSV, which opens in Excel, Numbers or Google Sheets.
Can parents fill this in for a child?
Yes - that's the normal case. Ask for the player's details and the parent's contact details on the same form, and keep the consent boxes for the adult to tick.
