A sports registration form is the same shape whatever the sport: who's playing, at what level, when they're available, what a coach needs to know, and what a parent has consented to.

Questions

Can I use this for adults and juniors?

Yes - the parent fields sit unused for adult players, or delete them and duplicate the form. Duplicating takes one click.

How do I collect subs?

Ask how they'd like to pay and invoice separately. Adding a discreet hardship question keeps families in the club who'd otherwise quietly leave.

Can I stop registrations when we're full?

Yes. Close the form and the link shows a short closed page, so nobody registers for a place that doesn't exist. Reopen it in one click if space frees up.

Can I collect one registration per person or per group?

Either. Many people ask for the group size and the names in one text box; others publish the link and ask everyone to fill it in individually.

Can I get the list as a spreadsheet?

Yes - one click exports every response as CSV, which opens in Excel, Numbers or Google Sheets.

Can parents fill this in for a child?

Yes - that's the normal case. Ask for the player's details and the parent's contact details on the same form, and keep the consent boxes for the adult to tick.
