Swimming Lesson Registration Form
A swimming lesson registration form has to place a swimmer accurately, and ability in the water is not the same as confidence in it.
For clubs, teams, leagues and community sport organisers.
Ability and confidence are different questions
A child who can swim 25 metres but panics with their face in the water, and a child who can't swim but jumps in fearlessly, need different teaching and often different groups. Asking "can they swim?" collapses both into one useless answer.
So this template asks about distance, strokes and water confidence separately - including whether they're happy going underwater and out of their depth. That's what lets a teacher place a swimmer correctly in week one rather than week three.
Registration that closes itself
The worst version of this is taking registrations you can't honour. Close the form when you're full and everyone with the link sees that immediately, instead of signing up and finding out later.

Questions
How should we describe ability levels?
In concrete terms - metres swum, whether they can float, whether they'll put their face in. Badge names mean nothing to parents from another pool.
Should we ask about previous bad experiences?
Yes. A child who has had a frightening experience in water needs a teacher who knows before the first lesson, and parents will tell you if asked.
Can I stop registrations when we're full?
Yes. Close the form and the link shows a short closed page, so nobody registers for a place that doesn't exist. Reopen it in one click if space frees up.
Can I collect one registration per person or per group?
Either. Many people ask for the group size and the names in one text box; others publish the link and ask everyone to fill it in individually.
Can I get the list as a spreadsheet?
Yes - one click exports every response as CSV, which opens in Excel, Numbers or Google Sheets.
Can parents fill this in for a child?
Yes - that's the normal case. Ask for the player's details and the parent's contact details on the same form, and keep the consent boxes for the adult to tick.
