Sports Waiver Form
A sports waiver form should do two things properly: record an accurate health declaration, and set out the concussion protocol before anyone needs it.
For clubs, teams, leagues and community sport organisers.
The concussion paragraph is the one that matters
Most sports waivers are about liability. The section that actually protects a participant is the one nobody includes: what happens if someone takes a knock to the head - that they will be removed from play, that they won't return the same day, and that a medical clearance is needed before they train again.
Written down and acknowledged in advance, that protocol is much easier to enforce on a touchline against a player who wants to carry on and a parent who thinks they're fine.
A record you can actually rely on
Consent that was given verbally, or on a paper form in a drawer, is consent you cannot produce when it matters. What you need is the wording they agreed to, the answers they gave, and the date - all in one place.

Questions
Is a waiver legally binding?
It varies by jurisdiction, and you can rarely sign away liability for negligence. What a waiver reliably does is evidence that risks were explained - so keep the language plain and check it with your insurer.
Do we need one per season?
Yes, and re-declare health each time. An injury or diagnosis from last year is exactly what a coach needs to know this year.
Is this legal advice?
No. The wording is a practical starting point, not a legal document - have your own adviser check anything you rely on, especially where health, minors or money are involved.
Is a tick box a valid signature?
In many places a clear, dated, affirmative action is accepted, but it depends on your jurisdiction and what you're doing. If you need a drawn signature, print the response and sign it, or take the form as the pre-appointment record.
Can I keep a copy for the person who filled it in?
Yes - they see a confirmation on screen, and you can turn on an email so a copy of each submission lands in your inbox for your records.
Can parents fill this in for a child?
Yes - that's the normal case. Ask for the player's details and the parent's contact details on the same form, and keep the consent boxes for the adult to tick.
