Team Tryout Registration Form
A team tryout registration form needs enough to organise the session and enough to check eligibility. Age, current club, position and whether they can commit to the season.

Eligibility before ability, or the tryout was pointless
A player who trials brilliantly and turns out to be registered with another club, or outside the age band, or unavailable for half the fixtures, cannot be selected. Finding that out after the session wastes a place someone else wanted.
So this template asks about current registration, age band and season availability before anything else. It also asks what they're currently playing, which lets a coach group trialists sensibly rather than putting a first-timer against a county player.
Registration that closes itself
The worst version of this is taking registrations you can't honour. Close the form when you're full and everyone with the link sees that immediately, instead of signing up and finding out later.
Run by volunteers, on a phone, at the side of a pitch
Club admin happens in evenings and car parks. Whatever collects registrations, availability and subs has to work in thirty seconds on a phone, for a parent who has never heard of it and will never make an account.

Questions
Should we ask about current club registration?
Always. Transfer rules differ by sport and league, and a player who needs a transfer form is a player who needs it started now rather than after selection.
How do we handle players who aren't selected?
Tell them, personally, with something useful - a development squad, a session to work on, a date to try again. It's the difference between a player who returns and one who doesn't.
Can I stop registrations when we're full?
Yes. Close the form and the link shows a short closed page, so nobody registers for a place that doesn't exist. Reopen it in one click if space frees up.
Can I collect one registration per person or per group?
Either. Many people ask for the group size and the names in one text box; others publish the link and ask everyone to fill it in individually.
Can I get the list as a spreadsheet?
Yes - one click exports every response as CSV, which opens in Excel, Numbers or Google Sheets.
Can parents fill this in for a child?
Yes - that's the normal case. Ask for the player's details and the parent's contact details on the same form, and keep the consent boxes for the adult to tick.
