Basketball Registration Form

A basketball registration form has to sort players into teams that can actually play each other. Age, experience level and honest self-assessment do most of that.

For clubs, teams, leagues and community sport organisers.

Ask what level they've played at, in concrete terms

"Intermediate" means nothing across basketball. Someone who played high-school varsity and someone who plays weekly at a park both describe themselves that way, and putting them in the same recreational division ruins the league for one of them.

So this template asks about the level they last played at by name - school team, club league, recreational, never - which people answer accurately. It also asks about availability, because a league runs on players who can make the fixtures.

Know who's coming, and what they need

A name is the easy half. The awkward half - dietary needs, accessibility, emergency contacts, sizes - is the half that ruins a day when it's missing, and the half nobody thinks to send you.

So the form asks. Once, at registration, when the person actually knows the answer.

Run by volunteers, on a phone, at the side of a pitch

Club admin happens in evenings and car parks. Whatever collects registrations, availability and subs has to work in thirty seconds on a phone, for a parent who has never heard of it and will never make an account.

Questions

Should we ask for height?

For team balancing, it's useful - but ask it optionally and don't make it feel like a screening criterion. Experience matters far more at recreational level.

How do we balance teams fairly?

Ask for the level last played at plus years of experience, then have coaches assess in the first session. Self-reported ability alone is unreliable in both directions.

Can I stop registrations when we're full?

Yes. Close the form and the link shows a short closed page, so nobody registers for a place that doesn't exist. Reopen it in one click if space frees up.

Can I collect one registration per person or per group?

Either. Many people ask for the group size and the names in one text box; others publish the link and ask everyone to fill it in individually.

Can I get the list as a spreadsheet?

Yes - one click exports every response as CSV, which opens in Excel, Numbers or Google Sheets.

Can parents fill this in for a child?

Yes - that's the normal case. Ask for the player's details and the parent's contact details on the same form, and keep the consent boxes for the adult to tick.
