A tasting event registration form is the step before a big catering booking. Who's coming, which dishes they want to try, and what they can't eat - so the kitchen prepares the right things once.

A tasting is a production run for two
Tastings cost real money: ingredients, a chef's afternoon, and the opportunity cost of a Saturday slot. That's fine when the couple arrives having chosen five dishes; it's expensive when they arrive expecting to try everything.
So this template asks them to pick specific dishes, with a limit stated in the introduction. It also asks who's coming - a tasting for six needs different planning from a tasting for two - and confirms the date of their actual event, which is the thing you're really protecting.
One list, not five threads
Registrations that arrive by email, text and word of mouth become a headcount nobody trusts. You find out on the day that two people thought they were signed up and one wasn't.
This template makes one list, with the same details for every person, in the order they registered.
Allergies are not an optional field
Headcount, date and venue set the quote; allergies and dietary requirements decide whether the day goes well or badly. Both belong on the same form, asked before you cost the job, not confirmed by text the night before.
Written answers matter here more than most trades: when someone tells you in writing what they can't eat, there is a record of exactly what you were told.
A QR code away
Every form has a plain link, so a poster, a flyer or a QR code on the door is enough. Nobody has to find your website to sign up.

Questions
Should tastings be paid or free?
Whatever you decide, say it on the form. Many caterers charge and credit it against the booking, which filters out people who aren't seriously considering you.
How many dishes should I offer?
State the number in the introduction and let them choose - typically two starters, two mains and two desserts. An open choice leads to a mismatch of expectations at the table.
Can I stop registrations when we're full?
Yes. Close the form and the link shows a short closed page, so nobody registers for a place that doesn't exist. Reopen it in one click if space frees up.
Can I collect one registration per person or per group?
Either. Many people ask for the group size and the names in one text box; others publish the link and ask everyone to fill it in individually.
Can I get the list as a spreadsheet?
Yes - one click exports every response as CSV, which opens in Excel, Numbers or Google Sheets.
Can I take deposits through the form?
Not yet - payments aren't built in. Most caterers use the form to capture and confirm the brief, then invoice for the deposit, which also means you check the date is free before taking money.
