Tattoo Consent Form
A tattoo consent form should make the client confirm the design and the placement in their own words, on the day, before any needle touches skin.

Have them confirm the spelling and the placement themselves
The disputes that end badly are almost never about the quality of the work. They're about a misspelled word, a date with the wrong year, or a placement two inches from where the client thought it would be.
So this template asks the client to type the exact wording and confirm the placement, and asks them to tick that they have checked it. Once they've written it themselves, in their own words, on a dated record, that conversation is over before it starts.
Before the appointment, not in the waiting room
Paperwork at the door costs you the first ten minutes and gets rushed. Send the link when you confirm the booking and it arrives filled in, read properly, with time to flag anything that needs a conversation.
Age, aftercare and the design brief
Placement, size, reference images, skin history and age confirmation decide whether you can take a booking and how long to allow. They're also the questions that get skipped when everything happens in a DM.
A form gives you the brief in writing before you spend an evening drawing, and a dated record of the consent and age declaration you're required to hold.
Separate boxes, separate decisions
Bundling everything into one "I agree" makes the whole thing weaker. This template keeps the distinct agreements distinct, so someone can decline the optional parts and still be treated.

Questions
Should the client write the wording out?
Always, for any lettering, date or name. An artist transcribing from a message is one typo away from a free cover-up and a bad review.
Do we need a fresh consent each time?
Yes - each tattoo has its own design, placement and risks. Keep the health intake separate and update it, but consent is per piece.
Is this legal advice?
No. The wording is a practical starting point, not a legal document - have your own adviser check anything you rely on, especially where health, minors or money are involved.
Is a tick box a valid signature?
In many places a clear, dated, affirmative action is accepted, but it depends on your jurisdiction and what you're doing. If you need a drawn signature, print the response and sign it, or take the form as the pre-appointment record.
Can I keep a copy for the person who filled it in?
Yes - they see a confirmation on screen, and you can turn on an email so a copy of each submission lands in your inbox for your records.
Can I use this as the consent and age form?
Many studios do, with the declarations as separate tick boxes and the response kept as the dated record. Local rules on age verification and record-keeping vary - check what yours requires, and keep the wording specific.
