A minor piercing consent form has to establish parental authority beyond doubt, and record which sites the parent has actually agreed to.

Which sites, specifically - not blanket permission

A parent consenting to an ear piercing has not consented to a navel or a nipple, and the difference matters legally as well as ethically. Blanket consent from a parent who was thinking about earlobes is not consent at all.

So this template asks the parent to tick the specific sites permitted, and states plainly which sites are never performed on a minor regardless of consent. It also requires both the parent and the minor to be present with ID - because a signed note is trivially forged.

Say what they're agreeing to, in plain words

A consent form that nobody reads protects nobody. The statements in this template are short, specific and separate - so ticking one is a real decision rather than a formality.

Questions

What are the legal age limits?

They vary considerably by country and even by region, and some sites are prohibited on minors entirely. Check your local law and your insurer's requirements before using this - the form must reflect your rules, not ours.

Is a signed note from a parent enough?

Almost never. Best practice is the parent present in person with photo ID and proof of relationship. This template records that requirement explicitly.

Is this legal advice?

No. The wording is a practical starting point, not a legal document - have your own adviser check anything you rely on, especially where health, minors or money are involved.

Is a tick box a valid signature?

In many places a clear, dated, affirmative action is accepted, but it depends on your jurisdiction and what you're doing. If you need a drawn signature, print the response and sign it, or take the form as the pre-appointment record.

Can I keep a copy for the person who filled it in?

Yes - they see a confirmation on screen, and you can turn on an email so a copy of each submission lands in your inbox for your records.

Can I use this as the consent and age form?

Many studios do, with the declarations as separate tick boxes and the response kept as the dated record. Local rules on age verification and record-keeping vary - check what yours requires, and keep the wording specific.
