Minor Tattoo Consent Form
A minor tattoo consent form only exists where the law permits tattooing under 18 - and in many places it does not, at any age, with any consent. Check before you publish it.

In most places, the answer is no - and the form should say so
Tattooing anyone under 18 is a criminal offence in a large number of jurisdictions regardless of parental consent. In others it's permitted with parental attendance and ID. There is no single correct form, and using the wrong one is not a paperwork error.
So this template starts by stating that the studio follows local law, and requires the parent present in person with ID and proof of relationship. If your jurisdiction prohibits it, don't adapt this form - publish a page that says no and explains why.
Before the appointment, not in the waiting room
Paperwork at the door costs you the first ten minutes and gets rushed. Send the link when you confirm the booking and it arrives filled in, read properly, with time to flag anything that needs a conversation.
Age, aftercare and the design brief
Placement, size, reference images, skin history and age confirmation decide whether you can take a booking and how long to allow. They're also the questions that get skipped when everything happens in a DM.
A form gives you the brief in writing before you spend an evening drawing, and a dated record of the consent and age declaration you're required to hold.

Questions
Should we offer this at all?
Only if it is lawful where you operate and your insurer permits it. Many studios decline under-18 work regardless, which is a perfectly defensible policy - and simpler.
Is parental consent a defence if the law prohibits it?
No. Where the law sets a minimum age, consent does not override it, and the liability sits with the artist and the studio. Take legal advice before offering this.
Is this legal advice?
No. The wording is a practical starting point, not a legal document - have your own adviser check anything you rely on, especially where health, minors or money are involved.
Is a tick box a valid signature?
In many places a clear, dated, affirmative action is accepted, but it depends on your jurisdiction and what you're doing. If you need a drawn signature, print the response and sign it, or take the form as the pre-appointment record.
Can I keep a copy for the person who filled it in?
Yes - they see a confirmation on screen, and you can turn on an email so a copy of each submission lands in your inbox for your records.
Can I use this as the consent and age form?
Many studios do, with the declarations as separate tick boxes and the response kept as the dated record. Local rules on age verification and record-keeping vary - check what yours requires, and keep the wording specific.
