Ask how much freedom the artist actually has
The difference between a design an artist enjoys and one they redraw four times is usually unspoken: whether the client wants their idea executed exactly, or wants the artist's interpretation of it.
Both are legitimate, and neither works if it isn't agreed. So this template asks directly, on a scale from "follow my reference closely" to "do what you think is best". It also asks what absolutely must be in the piece - which is a much more useful answer than a description of the whole thing.
Why ask it all up front
Half-finished information is worse than none: it looks like you can begin, and then you can't. A client who fills in one page has told you enough to quote, schedule and prepare - without a single back-and-forth.
It also sets the tone. A form that asks precisely, and only once, reads as a business that has done this before. That impression is doing real work while your competitor is still emailing "quick question".
Age, aftercare and the design brief
Placement, size, reference images, skin history and age confirmation decide whether you can take a booking and how long to allow. They're also the questions that get skipped when everything happens in a DM.
A form gives you the brief in writing before you spend an evening drawing, and a dated record of the consent and age declaration you're required to hold.

Questions
Should this be separate from the booking form?
It's worth it for larger custom work. A booking form gets you a slot; a design consultation gets you a drawing worth showing, and they need different questions.
How do I handle clients with no clear idea?
Ask what they want it to feel like rather than look like, and ask for three images they're drawn to. Style preference is much easier to express visually than verbally.
Can I add or remove questions?
Yes - every question here is a starting point. Add your own, delete what doesn't apply, reorder by dragging, and make anything required or optional. Saving publishes the change immediately.
Where do the answers go?

Do I need a website?

Can I use this as the consent and age form?
Many studios do, with the declarations as separate tick boxes and the response kept as the dated record. Local rules on age verification and record-keeping vary - check what yours requires, and keep the wording specific.
