Piercing Consent Form
A piercing consent form has to cover the site, the jewellery and the healing. Most piercing problems are aftercare problems, and the form is where that expectation gets set.

Healing times, said before the piercing, not after
The commonest piercing complaint is that it hasn't healed - three weeks into a cartilage piercing that takes six months. Nobody was misled; nobody was told, because the conversation happened in a busy studio and the number went past.
So this template states the healing time for the chosen site and asks the client to acknowledge it. It also asks about fainting history and whether they've eaten - the two questions that prevent the most common incident in a piercing studio.
Say what they're agreeing to, in plain words
A consent form that nobody reads protects nobody. The statements in this template are short, specific and separate - so ticking one is a real decision rather than a formality.

Age, aftercare and the design brief

Questions
Should we ask if they've eaten?
Yes. Fainting is the most common adverse event in piercing, and low blood sugar is the biggest predictor. It's one question and it prevents a genuine injury.
What jewellery should we recommend?
Ask about metal allergies and recommend implant-grade titanium for initial piercings. Recording the choice matters if a reaction occurs later.
Is this legal advice?
No. The wording is a practical starting point, not a legal document - have your own adviser check anything you rely on, especially where health, minors or money are involved.
Is a tick box a valid signature?
In many places a clear, dated, affirmative action is accepted, but it depends on your jurisdiction and what you're doing. If you need a drawn signature, print the response and sign it, or take the form as the pre-appointment record.
Can I keep a copy for the person who filled it in?
Yes - they see a confirmation on screen, and you can turn on an email so a copy of each submission lands in your inbox for your records.
Can I use this as the consent and age form?
Many studios do, with the declarations as separate tick boxes and the response kept as the dated record. Local rules on age verification and record-keeping vary - check what yours requires, and keep the wording specific.
