Clients don't think of fish oil, aspirin or a course of steroids as relevant to a tattoo. All three affect bleeding or healing, and an artist working on skin that won't hold ink properly produces a result everyone blames on the artist.

So this template asks by name rather than generally - blood thinners, retinoids, immunosuppressants, recent antibiotics - and asks how previous tattoos healed. That last answer is the best available predictor of how this one will go.

Questions

How long should we keep these records?

As long as your local rules and insurer require - often several years. Responses are dated and export as a spreadsheet, which is usually what an inspection asks to see.

Should we re-take it for repeat clients?

Send a shorter health update each visit. Health and medication change; the rest doesn't.

Can I add or remove questions?

Yes - every question here is a starting point. Add your own, delete what doesn't apply, reorder by dragging, and make anything required or optional. Saving publishes the change immediately.

Where do the answers go?

Many studios do, with the declarations as separate tick boxes and the response kept as the dated record. Local rules on age verification and record-keeping vary - check what yours requires, and keep the wording specific.
