Licensing and insurance, before the portfolio
A guest artist working in your studio is working under your registration and, in some places, your liability. An artist without their own insurance and current infection control training is a risk to your licence, however good the portfolio.
So this template asks for registration numbers, insurance and training dates first. It also asks about their setup - what they bring and what they need - because a guest expecting a full station and finding a chair is a bad week for both of you.
Screen before you spend time
Most of the work in an application process is on the ones that were never eligible. Asking the disqualifying questions first - dates, requirements, basic facts - means you spend your reading time on the real candidates.
Age, aftercare and the design brief
Placement, size, reference images, skin history and age confirmation decide whether you can take a booking and how long to allow. They're also the questions that get skipped when everything happens in a DM.

Questions
What licensing should we ask about?
Whatever your jurisdiction requires - local authority registration, infection control certification, bloodborne pathogens training - with dates. Requirements differ; ask for yours explicitly.
Should we ask about their following?
Ask for their portfolio and social handle. A guest spot only works commercially if they can fill their own books, so audience is relevant - but the portfolio and the licensing come first.
Can applicants attach a CV or document?
Not yet - file uploads are on the roadmap. Most people ask for a link (to a portfolio, a CV in cloud storage, or a profile) which works today and keeps the form fast.
Can I close applications on a deadline?
Yes. Close the form and anyone with the link sees a short closed page instead of the questions, so nobody fills in an application that can't be considered.
Can more than one person review them?
Yes - invite your team into the same account and they'll see the same responses. You can also export to a spreadsheet and score them wherever you like.
Can I use this as the consent and age form?
Many studios do, with the declarations as separate tick boxes and the response kept as the dated record. Local rules on age verification and record-keeping vary - check what yours requires, and keep the wording specific.
