Student Emergency Contact Form
A student emergency contact form is a document that has to be right and has to be current. Three contacts in priority order, the medical facts, and the doctor's number - refreshed every year.
For schools, colleges, tutors and education administrators.
Priority order, because the first number rings out
The value of an emergency contact list is entirely in the second and third entries. The first is at work with their phone on silent - that's not a failure of the system, it's the normal case.
So this template asks for three contacts explicitly and asks parents to put them in the order they should be tried. It also asks for the relationship, because "try Dave" is much less useful at 2pm than "grandfather, lives five minutes away".
Registration that closes itself
The worst version of this is taking registrations you can't honour. Close the form when you're full and everyone with the link sees that immediately, instead of signing up and finding out later.
Term-time admin, with permission slips that arrive
A trip letter sent home in a bag comes back late, damp or not at all. The same request as a link in an email or text is answered the same evening, by both parents, with the emergency contact and the allergy filled in.
And you get a list instead of a pile: who has consented, who hasn't, and what each child needs - exportable to whatever the office already runs on.

Questions
How do we keep this up to date?
Send the same link at the start of every year, and after any house move. Responses are dated, so the office can see instantly which records are stale.
Can students fill this in themselves?
For older students, often yes - but the medical consent should come from a parent or guardian. Many schools send it to parents and copy the student.
Can I stop registrations when we're full?
Yes. Close the form and the link shows a short closed page, so nobody registers for a place that doesn't exist. Reopen it in one click if space frees up.
Can I collect one registration per person or per group?
Either. Many people ask for the group size and the names in one text box; others publish the link and ask everyone to fill it in individually.
Can I get the list as a spreadsheet?
Yes - one click exports every response as CSV, which opens in Excel, Numbers or Google Sheets.
Do parents need an account or an app?
No. They tap a link and answer. Only staff need an account, and you can invite colleagues into the same one so the office and the trip lead see the same responses.
