School Transfer Request Form

Transfer requests come from house moves, from a child who is unhappy, and sometimes from something serious. The reason genuinely matters for prioritisation and for the conversation that follows - but a required free-text box asking a parent to justify themselves reads badly.

So this template offers a short list of reasons plus an optional space to explain in confidence. Families who want to say more can; families who don't aren't forced to write about bullying in a text box before anyone has spoken to them.

Questions

Does this replace the local authority process?

No - in most places transfers run through a council or district process, and the form should say so with a link. Use it to open the conversation and gather what you need for your own records.

Should we ask about additional needs?

Yes, and say why: knowing about an EHCP, IEP or support plan early is what lets you answer honestly about whether you can meet a child's needs.

Can applicants attach a CV or document?

Not yet - file uploads are on the roadmap. Most people ask for a link (to a portfolio, a CV in cloud storage, or a profile) which works today and keeps the form fast.

Can I close applications on a deadline?

Yes. Close the form and anyone with the link sees a short closed page instead of the questions, so nobody fills in an application that can't be considered.

Can more than one person review them?

Yes - invite your team into the same account and they'll see the same responses. You can also export to a spreadsheet and score them wherever you like.

Do parents need an account or an app?

No. They tap a link and answer. Only staff need an account, and you can invite colleagues into the same one so the office and the trip lead see the same responses.
