School Club Signup Form

Ranked choices, not a wish list
Every club sign-up produces the same collision: three popular clubs on Tuesday and one child who wants all of them. A form that collects a flat list of wants leaves the office guessing which to allocate.
Asking children and parents to put their choices in order solves it in one step. This template uses a drag-to-order question - which degrades to plain number dropdowns without JavaScript, so it works on the oldest device in the house.

Questions
Can we cap each club?
Not automatically - but with ranked preferences you can allocate fairly and tell people what they got. When a club is full, remove it from the list and the live form updates immediately.
Should the child or the parent fill it in?
The parent, because of the consent and collection questions - but write the club descriptions for the child. They're the one choosing.
Can I close sign-ups when I have enough?
Yes - close the form and the link shows a short closed page instead of the questions. Reopen it whenever you like.
Can I email everyone who signed up?
Export the responses as a spreadsheet and use whatever you already send email with. ava42 collects the list; it isn't a mailing tool.
Do people need an account?
No. Only you have an account. Anyone with the link can fill the form in, with no sign-up and no app.
Do parents need an account or an app?
No. They tap a link and answer. Only staff need an account, and you can invite colleagues into the same one so the office and the trip lead see the same responses.
