Parent Consent Form
A parent consent form works best as a list of separate yes-or-no decisions. A trip, a photograph, a plaster and a coach journey are different things, and a parent may say yes to three of them.

Questions
Is an electronic consent acceptable?
For most school permissions, a clear dated affirmative action from a named parent is what's needed, and that's what a submission is. Check your own policy and local guidance for anything involving medical treatment or overnight stays.
Can both parents respond?
Yes - send the link to both. Each submission is separate and dated, which is useful where parents live apart and consent needs to be evidenced.
Is this legal advice?
No. The wording is a practical starting point, not a legal document - have your own adviser check anything you rely on, especially where health, minors or money are involved.
Is a tick box a valid signature?
In many places a clear, dated, affirmative action is accepted, but it depends on your jurisdiction and what you're doing. If you need a drawn signature, print the response and sign it, or take the form as the pre-appointment record.
Can I keep a copy for the person who filled it in?

Do parents need an account or an app?
