After School Program Registration Form
An after school program registration form has to be exact about two things: which days a child attends, and who is allowed to take them home. Everything else is detail.
For schools, colleges, tutors and education administrators.
A pattern of days, not a yes or no
After-school clubs are staffed by ratio, per day. "Yes, we'd like a place" is not bookable - Mondays and Wednesdays is. And a child who turns up on a day they're not registered for is a safeguarding problem, not an administrative one.
So the days question is required and specific here, with collection arrangements immediately after. Export the responses and you have the register for each day of the week, with the named adults who may collect each child.
One list, not five threads
Registrations that arrive by email, text and word of mouth become a headcount nobody trusts. You find out on the day that two people thought they were signed up and one wasn't.

Questions
Can parents book ad-hoc days?
Add a question for occasional days alongside the regular pattern. Most clubs run a separate short form for ad-hoc bookings so the register stays clean.
How do we handle payment?
Collect registrations here and invoice through whatever you already use. Asking how they intend to pay - including childcare vouchers or tax-free accounts - saves a round of emails.
Can I stop registrations when we're full?
Yes. Close the form and the link shows a short closed page, so nobody registers for a place that doesn't exist. Reopen it in one click if space frees up.
Can I collect one registration per person or per group?
Either. Many people ask for the group size and the names in one text box; others publish the link and ask everyone to fill it in individually.
Can I get the list as a spreadsheet?
Yes - one click exports every response as CSV, which opens in Excel, Numbers or Google Sheets.
Do parents need an account or an app?
