Sponsor Application Form

A sponsor application form should let a company tell you what success looks like for them. Package and budget are the easy half; what they actually want out of it is what makes the relationship work.

For wedding planners, venues, event coordinators and suppliers.

Objectives, so you can deliver something they'll renew

A sponsor who wanted recruitment leads and got brand awareness will not sponsor again, even if you delivered everything in the package. The package was never the point - it was a proxy for an outcome nobody wrote down.

So this template asks what they're trying to achieve before it asks which tier they want. That's the information that lets you suggest the right package, and the information you'll use in six months to show them what they got.

Screen before you spend time

Most of the work in an application process is on the ones that were never eligible. Asking the disqualifying questions first - dates, requirements, basic facts - means you spend your reading time on the real candidates.

Dates, headcounts and the details nobody volunteers

Every event enquiry needs the same four facts - date, numbers, venue, budget - and almost none arrive with them. Asking on a form means your first reply can be availability and a price rather than a questionnaire.

The awkward details are easier on a form too: dietary requirements, accessibility, plus ones and the timings that only matter on the day.

Long enough to decide, short enough to finish

Applications fail in two directions: too short to judge, or so long that good candidates give up. This template asks what a decision genuinely needs, split into steps so it never looks like a wall.

Yours to change, immediately

Nothing here is fixed. Rewrite a question, reorder them by dragging, add your brand colour, delete half of it. Save and the live form updates - there's no separate publish step and no deploy to wait for.

Questions

Should I list packages and prices on the form?

Yes - put them in the introduction and ask which they want. Companies self-select much more accurately than they do in a conversation, and it filters out the ones with no budget.

Can I ask for logo files?

Ask for a link to a brand folder rather than an attachment - it's how marketing teams already work, and it gets you the right file formats.

Can applicants attach a CV or document?

Not yet - file uploads are on the roadmap. Most people ask for a link (to a portfolio, a CV in cloud storage, or a profile) which works today and keeps the form fast.

Can I close applications on a deadline?

Yes. Close the form and anyone with the link sees a short closed page instead of the questions, so nobody fills in an application that can't be considered.

Can more than one person review them?

Yes - invite your team into the same account and they'll see the same responses. You can also export to a spreadsheet and score them wherever you like.

Can I use one form for several events?

Ask which event in a dropdown and filter the responses by it, or duplicate the form for each one so each has its own link and its own list. Duplicating takes a click.
