Yoga Class Registration Form
A yoga class registration form should ask which style someone has done, not just how long they've practised. Two years of restorative yoga is not preparation for an ashtanga class.

Style matters more than years
"Intermediate" means nothing across styles. Someone confident in hot vinyasa may have never held a long passive stretch; a dedicated Iyengar practitioner may not want a sixty-minute flow. Both would call themselves experienced.
So this template asks which styles they've practised and what they're looking for from a class, alongside injuries and pregnancy. It means a first class is a good fit rather than a gamble - and a teacher can offer the right variations before anyone is on a mat.
One list, not five threads
Registrations that arrive by email, text and word of mouth become a headcount nobody trusts. You find out on the day that two people thought they were signed up and one wasn't.

Screening questions you can't afford to skip
Injuries, conditions, medication and goals decide the programme and the liability. Asked in a doorway they get half an answer; asked on a form before the first session they get a real one.

Questions
Should I ask about pregnancy?
Yes, and separately from other conditions - it changes the practice throughout, not just the intensity, and most teachers want to know the trimester.
Can I use one form for drop-ins and courses?
Ask which they want as the first question. Course registrations usually need more detail; drop-ins need the health declaration and little else.
Can I stop registrations when we're full?
Yes. Close the form and the link shows a short closed page, so nobody registers for a place that doesn't exist. Reopen it in one click if space frees up.
Can I collect one registration per person or per group?
Either. Many people ask for the group size and the names in one text box; others publish the link and ask everyone to fill it in individually.
Can I get the list as a spreadsheet?

Can I include a health questionnaire and a waiver together?
Yes - many trainers use one form with the screening questions first and the agreement as separate tick boxes at the end. Keep the wording specific, and have an adviser check anything you rely on.
