Nutrition Coaching Intake Form

A nutrition coaching intake form asks about a normal day, not an ideal one. What they actually eat, when, and around what - because a plan built on a description of good intentions fails in week two.

Ask what yesterday looked like

"Describe your diet" produces an aspirational answer. "What did you eat yesterday, from waking up to going to bed?" produces the truth, including the bit at 9pm that nobody thinks counts.

So this template asks for yesterday specifically, plus the shape of the day around it - work hours, who cooks, how often food is eaten out. Those constraints are what make a plan followable, and they're invisible if you only ask about food.

What a good intake form saves you

The first message from a new client almost never contains what you need. You reply asking one question; they answer a different one; a week later you still can't start. Every round trip is a day, and the clients who go quiet are usually the ones who got tired of answering.

One form ends that. Ask everything once, get a complete picture, and start the same day. The questions in this template are the ones you end up asking anyway - the form just gets them asked up front.

Screening questions you can't afford to skip

Injuries, conditions, medication and goals decide the programme and the liability. Asked in a doorway they get half an answer; asked on a form before the first session they get a real one.

It also lets you triage: the person who needs a GP referral, the returning client, the absolute beginner - each identified before they're standing in front of you.

Fast enough that people finish it

Abandonment is mostly a speed problem. A form that takes a second to appear and submits in one request gets finished; one that loads a framework first does not. This is a single self-contained page - no scripts to download, nothing to wait for.

Questions

Do I need to ask about medical history?

Ask enough to know when to refer - diagnosed conditions, medication, disordered eating history. Nutrition coaching is not dietetics, and knowing where your scope ends is part of practising responsibly.

Should I ask for weight?

Only if you'll use it, and offer a "prefer not to say" option. For many clients the number is loaded, and measurements or how clothes fit work just as well for tracking progress.

Can I add or remove questions?

Yes - every question here is a starting point. Add your own, delete what doesn't apply, reorder by dragging, and make anything required or optional. Saving publishes the change immediately.

Where do the answers go?

Into your dashboard, where you can read, filter and export them as a spreadsheet. You can also have every response emailed to you the moment it arrives, or as a daily or weekly digest.

Do I need a website?

No. Every form gets its own link you can paste into an email signature, a text message, a QR code or a social profile. If you do have a site, you can link to it from anywhere on it.

Can I include a health questionnaire and a waiver together?

Yes - many trainers use one form with the screening questions first and the agreement as separate tick boxes at the end. Keep the wording specific, and have an adviser check anything you rely on.
