Fitness Waiver Form
A fitness waiver form is a health declaration plus an acknowledgement of risk, dated automatically, in the member's own words. Both halves matter; most waivers only do the second.

A declaration is worth more than a disclaimer
The clause saying you're not liable is the part gyms focus on and the part that matters least. What actually protects a member - and therefore you - is an accurate health declaration you read before they trained.
So this template puts the health questions first and makes them specific: conditions, medication, injuries, pregnancy. The risk acknowledgement follows as separate ticks, and the emergency contact is required, because that is the field someone will actually need one day.

Questions
Is a digital waiver enforceable?
A dated, affirmative submission from a named person is what most insurers want to see, but this varies by jurisdiction - check with yours. Keep the wording plain, because a clause nobody could understand is the easiest one to challenge.
How often should members re-declare?
Annually, and after any injury or pregnancy. Because responses are dated you can see who is overdue, which is more than most paper systems manage.
Is this legal advice?
No. The wording is a practical starting point, not a legal document - have your own adviser check anything you rely on, especially where health, minors or money are involved.
Is a tick box a valid signature?
In many places a clear, dated, affirmative action is accepted, but it depends on your jurisdiction and what you're doing. If you need a drawn signature, print the response and sign it, or take the form as the pre-appointment record.
Can I keep a copy for the person who filled it in?
Yes - they see a confirmation on screen, and you can turn on an email so a copy of each submission lands in your inbox for your records.
Can I include a health questionnaire and a waiver together?
Yes - many trainers use one form with the screening questions first and the agreement as separate tick boxes at the end. Keep the wording specific, and have an adviser check anything you rely on.
