Personal Training Intake Form
A personal training intake form has to establish what a body can currently do before anyone writes a programme. Training history, injuries, screening - then goals and availability.
For gyms, personal trainers, studios and wellness coaches.
Injury history is the programme
A shoulder that dislocated twice in 2019 rules out certain movements permanently, and no amount of enthusiasm changes that. Yet injury history is usually collected as one line on a form and never read again.
So this template asks what, when, whether it was diagnosed and what still aggravates it. That last question is the useful one - it tells you the movement to avoid in session one, which is the difference between a client who progresses and a client who flares up and quietly stops booking.
Why ask it all up front
Half-finished information is worse than none: it looks like you can begin, and then you can't. A client who fills in one page has told you enough to quote, schedule and prepare - without a single back-and-forth.
It also sets the tone. A form that asks precisely, and only once, reads as a business that has done this before. That impression is doing real work while your competitor is still emailing "quick question".
Screening questions you can't afford to skip
Injuries, conditions, medication and goals decide the programme and the liability. Asked in a doorway they get half an answer; asked on a form before the first session they get a real one.
It also lets you triage: the person who needs a GP referral, the returning client, the absolute beginner - each identified before they're standing in front of you.
It works on a phone, on a bad signal
People fill these in from a car park, a waiting room, a sofa at 11pm. This form is one small page that opens the instant they tap the link and submits without needing anything clever in their browser - so an answer sent on two bars still reaches you.

Questions
Should this include the waiver?
Many trainers combine them - screening questions, then the acknowledgements. Others keep the waiver separate so it can be re-signed annually without redoing the whole intake.
How much detail on training history?
Enough to know where to start: what they've done, how recently, and what they can already do. "Never lifted" and "lifted for five years, stopped two years ago" are completely different starting points.
Can I add or remove questions?
Yes - every question here is a starting point. Add your own, delete what doesn't apply, reorder by dragging, and make anything required or optional. Saving publishes the change immediately.
Where do the answers go?
Into your dashboard, where you can read, filter and export them as a spreadsheet. You can also have every response emailed to you the moment it arrives, or as a daily or weekly digest.
Do I need a website?
No. Every form gets its own link you can paste into an email signature, a text message, a QR code or a social profile. If you do have a site, you can link to it from anywhere on it.
Can I include a health questionnaire and a waiver together?
Yes - many trainers use one form with the screening questions first and the agreement as separate tick boxes at the end. Keep the wording specific, and have an adviser check anything you rely on.
