Pilates Class Registration Form

A Pilates class registration form has to place someone in the right class. Level, injury history and whether they've used a reformer before - because a beginner in an intermediate class gets nothing out of the hour.

Level, and honesty about level

Everyone puts themselves in the intermediate class. Then they can't hold a plank and spend fifty minutes being corrected, which is nobody's idea of a good first session.

Describing what each level actually involves - and asking specifically about reformer versus mat experience - gets far more accurate self-placement. This template also asks about pregnancy and recent surgery separately, because both change the repertoire completely rather than just the intensity.

Know who's coming, and what they need

A name is the easy half. The awkward half - dietary needs, accessibility, emergency contacts, sizes - is the half that ruins a day when it's missing, and the half nobody thinks to send you.

So the form asks. Once, at registration, when the person actually knows the answer.

Screening questions you can't afford to skip

Injuries, conditions, medication and goals decide the programme and the liability. Asked in a doorway they get half an answer; asked on a form before the first session they get a real one.

It also lets you triage: the person who needs a GP referral, the returning client, the absolute beginner - each identified before they're standing in front of you.

How do I stop everyone booking the wrong level?

Describe each level in terms of what you'll be asked to do, not in adjectives. "Comfortable holding a plank for 30 seconds" places people far better than "intermediate".

Do I need separate forms for mat and reformer?

One form with an equipment question works well - reformer experience is the useful distinction, and asking it here saves an awkward first session.

Can I stop registrations when we're full?

Yes. Close the form and the link shows a short closed page, so nobody registers for a place that doesn't exist. Reopen it in one click if space frees up.

Can I collect one registration per person or per group?
