Fitness Challenge Signup Form
A fitness challenge signup form has to do two things at once: get people excited enough to commit, and screen the ones who shouldn't be doing a 30-day challenge unsupervised.
For gyms, personal trainers, studios and wellness coaches.
Screen without killing the momentum
Challenges convert on impulse. A long medical questionnaire at the moment of enthusiasm loses most of the people you wanted - but skipping the screening entirely means someone with an unmanaged heart condition doing burpees on day one.
The compromise this template uses: three short screening questions with a clear "speak to us first" outcome, placed after the fun part. People who tick a contraindication get a conversation rather than a rejection, and the other 95% are signed up in ninety seconds.
Make signing up the easy part
Enthusiasm has a short half-life. If signing up means finding an email address, writing a message and waiting, most of the people who meant to do it won't.
A link they can tap and finish in a minute converts the moment they're actually interested - which is usually the moment they hear about it.
Screening questions you can't afford to skip
Injuries, conditions, medication and goals decide the programme and the liability. Asked in a doorway they get half an answer; asked on a form before the first session they get a real one.
It also lets you triage: the person who needs a GP referral, the returning client, the absolute beginner - each identified before they're standing in front of you.
Works on the phone in their hand
One small page that opens instantly and submits in one request. That matters more than it sounds: most people are signing up on a phone, often on a poor connection.
One minute to make it your own
Every label, option and placeholder on this template is editable, and you can add whatever this template doesn't ask. Save to publish, share the link, and export the responses whenever you need them in a spreadsheet.

Questions
Should I make the challenge team-based?
Teams finish at much higher rates than individuals. Ask whether they're joining alone or with a team, and offer to place solo entrants - that single option converts the people who'd otherwise not start.
Do I need a waiver as well?
For anything physical, yes - either as a section here or as a separate form. Keep the wording specific to the activity rather than generic.
Can I close sign-ups when I have enough?
Yes - close the form and the link shows a short closed page instead of the questions. Reopen it whenever you like.
Can I email everyone who signed up?
Export the responses as a spreadsheet and use whatever you already send email with. ava42 collects the list; it isn't a mailing tool.
Do people need an account?
No. Only you have an account. Anyone with the link can fill the form in, with no sign-up and no app.
Can I include a health questionnaire and a waiver together?
Yes - many trainers use one form with the screening questions first and the agreement as separate tick boxes at the end. Keep the wording specific, and have an adviser check anything you rely on.
