Someone books a wellness consultation about fatigue. The interesting information is usually next to it - five hours of sleep, high stress, no daylight before noon. None of that comes up if you only ask about the presenting complaint.
So this template asks for quick ratings across five areas plus what they've already tried. Reading it takes two minutes and gives you the shape of the problem, which means the consultation can be about the plan rather than the diagnosis.
What a good intake form saves you
The first message from a new client almost never contains what you need. You reply asking one question; they answer a different one; a week later you still can't start. Every round trip is a day, and the clients who go quiet are usually the ones who got tired of answering.
One form ends that. Ask everything once, get a complete picture, and start the same day. The questions in this template are the ones you end up asking anyway - the form just gets them asked up front.
Screening questions you can't afford to skip
Injuries, conditions, medication and goals decide the programme and the liability. Asked in a doorway they get half an answer; asked on a form before the first session they get a real one.

Questions
How do I keep this from feeling clinical?
Use scales rather than long questionnaires, and say in the introduction that you're looking for the overall picture. People answer ten scale questions happily and abandon three essay boxes.
When should I refer someone on?
Whenever an answer suggests something medical. Ask about diagnosed conditions and medication, and be explicit on the form that this isn't medical advice.
Can I add or remove questions?
Yes - every question here is a starting point. Add your own, delete what doesn't apply, reorder by dragging, and make anything required or optional. Saving publishes the change immediately.
Where do the answers go?

Do I need a website?

Can I include a health questionnaire and a waiver together?
Yes - many trainers use one form with the screening questions first and the agreement as separate tick boxes at the end. Keep the wording specific, and have an adviser check anything you rely on.
