Shipping Request Form
A shipping request form is the internal booking that tells a despatch team what to send, where, on which service, and with what paperwork.
For hauliers, couriers, freight forwarders and fleet operators.
The paperwork is the part that goes wrong
A shipment held at customs, or refused by a carrier, is almost always a documentation failure: no commercial invoice, a missing EORI, a value declared as zero for a sample. None of it is hard - it's just never asked for at the point of request.
So this template asks who is providing the invoice, what value to declare, and whether the goods are a sale, a sample or a return. Those three answers prevent most avoidable delays, and they cost the requester about twenty seconds.
Orders you don't have to decode
Orders taken by phone, text and social message all arrive in different shapes, and the missing detail is always the one you need to start. Then you're calling back, and they're not answering.
One form means every order arrives complete, in the same shape, with a written record of exactly what was asked for.
Dimensions, weight and dates - or it isn't a quote
A shipping enquiry without dimensions, weight and both postcodes cannot be priced, so it becomes a phone call. On a form, those are four fields and a dropdown.
The same applies to bookings and claims: the specifics are the entire message, and asking for them in a fixed order gives you something a dispatcher can act on immediately.

Website, bio, QR code on the counter, reply to a message. Same form, same list, whichever way the order arrives.

Questions
Should this be an internal form or customer-facing?
Usually internal - it's the request a sales or warehouse team raises. Customer-facing shipping usually needs fewer questions and more explanation.
How do we handle recurring shipments?
Ask whether it's a repeat and let the requester paste a previous reference. It's faster than re-entering an address they've sent to fifty times.
Can I take payment on the form?
Not yet. Most people use this to capture the order and then invoice or take payment on collection - which also means you can confirm availability before charging anyone.
Can I limit what people can order?
Yes. Options are lists you control, so only what you actually sell appears - and you can close the form when you're at capacity.
Will I be told about new orders straight away?
Yes, if you turn on live notifications: each order is emailed to you as it arrives. Otherwise they collect in your dashboard.
Can drivers submit forms from a phone?
Yes - that's the main way these get used. One small page, works on a poor signal, no app to install on anyone's personal phone.
