Delivery Request Form
A delivery request form is about the last fifty metres. The address gets a van to the street; access notes get the goods through the door.
For hauliers, couriers, freight forwarders and fleet operators.
Failed deliveries are almost always an access problem
The cost of a redelivery is the whole margin on the first one. And the reason for a failure is rarely the address - it's a locked gate, a buzzer that doesn't work, a lift that's out, nobody in, or a stairwell too narrow for the item.
So this template asks about floors, lifts, parking and what to do if nobody's home - and asks for an alternative instruction rather than leaving the driver to decide on the doorstep. It's the difference between one visit and three.
Fewer no-shows, fewer surprises
No-shows and "I thought I booked the other thing" both come from the same place: a booking nobody wrote down properly. When the customer types it themselves, there's a record, in their words, of exactly what was asked for.
Dimensions, weight and dates - or it isn't a quote
A shipping enquiry without dimensions, weight and both postcodes cannot be priced, so it becomes a phone call. On a form, those are four fields and a dropdown.
The same applies to bookings and claims: the specifics are the entire message, and asking for them in a fixed order gives you something a dispatcher can act on immediately.

Questions
Should I let customers authorise leaving items unattended?
Ask, and record the answer - it's your evidence if something goes missing. Say clearly whether your insurance covers items left unattended.
How specific should the timing question be?
Offer windows rather than times. Promising 2pm and arriving at 4 costs more goodwill than promising an afternoon and arriving at 2.
Does this book the slot automatically?
No - it collects a request, and you confirm it. That's deliberate: real diaries have exceptions, and a form that promises a slot you can't honour costs more than one that asks.
Can I limit the options people can pick?
Yes. The service and time-of-day lists are yours to edit, so people can only choose from what you actually offer. You can also close the form when you're full.
Will I know straight away?

Can drivers submit forms from a phone?
